Overview

The stratum corneum extracellular lipid matrix is the primary physical and chemical barrier of the human skin, located in the outermost layer of the epidermis (Elias, 2005, PMID: 15930091). It consists of a highly organized mixture of ceramides, free fatty acids, and cholesterol arranged in lamellar structures that surround the corneocytes, often described by the 'bricks and mortar' model (Proksch et al., 2008, PMID: 18306158). This lipid matrix is essential for preventing excessive transepidermal water loss and protecting the body from environmental insults, including pathogens and allergens (van Smeden et al., 2014, PMID: 24553931). In various dermatological conditions such as atopic dermatitis and psoriasis, the composition and organization of these lipid domains are significantly altered, leading to barrier dysfunction and increased sensitivity (Bouwstra & Ponec, 2006, PMID: 16919013).
