Overview

The stratum corneum (SC) intercellular lipid matrix and corneodesmosomal structures represent the primary physical and permeability barrier of the skin, functioning as the mortar and rivets in the classic brick-and-mortar model (Proksch et al., 2008). The lipid matrix is composed of a unique mixture of ceramides, cholesterol, and free fatty acids organized into lamellar phases that prevent transepidermal water loss and the entry of exogenous substances (Bouwstra et al., 2006). Corneodesmosomes are specialized intercellular junctions that provide mechanical cohesion between corneocytes; their programmed proteolysis by enzymes like kallikreins is essential for healthy desquamation (Ishida-Yamamoto et al., 2011). Defects in these structures, such as reduced ceramide levels or premature corneodesmosome degradation, are central to the pathogenesis of atopic dermatitis, psoriasis, and various ichthyoses (StatPearls, 2023). Therapeutic interventions target these structures through the application of barrier-repair emollients, keratolytics that promote desquamation, or penetration enhancers that temporarily disrupt lipid packing to facilitate drug delivery.
