Overview

The stratum corneum lipid–protein complex is the functional ensemble of lipids (primarily ceramides, cholesterol, and free fatty acids) and specialized proteins (including keratin, filaggrin, involucrin, and loricrin) that together form the skin’s outer permeability barrier. This highly organized matrix is responsible for minimizing water loss, protecting against environmental insults, and contributing to innate cutaneous immunity. The key architectural elements are intercellular lamellar lipid bilayers and corneocyte envelopes densely packed with keratin and other structural proteins. Lipid composition and organization are dynamically regulated and vary with age, anatomical site, and disease state. Deficiencies or imbalances in SC lipids or proteins can result in impaired barrier function, leading to conditions such as atopic dermatitis, ichthyosis, and increased infection risk. While not a direct therapeutic target, interventions often focus on restoring or mimicking SC lipid/protein balance to manage skin diseases and enhance skin barrier function.
