The stratum corneum lipid and protein matrix is the extracellular matrix of the stratum corneum, comprised primarily of highly ordered lipid lamellae (mainly ceramides, cholesterol, and free fatty acids) and structural proteins (such as involucrin and loricrin) organized around corneocytes, along with enzymes and antimicrobial peptides[2][3][7]. This matrix forms the principal skin barrier that prevents excessive water loss, limits entry of environmental substances, and contributes to skin health and defense. Defective organization or composition of this matrix leads to impaired barrier function, characteristic of conditions such as atopic dermatitis, ichthyosis, and dry skin[2][4][5][6][9]. The matrix is not a druggable molecular target per se but rather a structure modulated by topical therapies, especially those that aim to restore lipid content or barrier properties.
