Overview

The stratum corneum lipid lamellae are highly organized, multi-layered structures formed from ceramides, cholesterol, and free fatty acids in approximately equimolar ratios. These lipids are secreted from lamellar bodies in keratinocytes and arrange into broad extracellular sheets, creating a hydrophobic barrier that is critical in preventing water loss and providing defense against environmental insults. Two major lamellar periodicities are recognized: short periodicity phase (SPP, ~6 nm) and long periodicity phase (LPP, ~13 nm); proper organization and packing of these structures is vital for skin barrier function. Disruption or deficiency in their composition or structure is associated with barrier dysfunction and various skin diseases, including atopic dermatitis and ichthyosis[1][3][4][5][7][8]. The lamellae are not a therapeutically targeted molecule but can be supported or mimicked by topical therapies to restore barrier function.
