The stratum corneum protein and lipid matrix represents the primary physical barrier of the human body, often conceptualized through the 'brick and mortar' model (StatPearls, NBK513299). In this model, protein-rich corneocytes act as the bricks, while a complex extracellular matrix of lipids—primarily ceramides, cholesterol, and free fatty acids—serves as the mortar (PubMed, 18254808). This structure is essential for maintaining homeostasis by preventing excessive transepidermal water loss and shielding the body from environmental pathogens, allergens, and chemical irritants (Journal of Investigative Dermatology, 2012). Dysregulation of this matrix, whether through genetic mutations in proteins like filaggrin or alterations in lipid composition, is a hallmark of inflammatory skin diseases such as atopic dermatitis and psoriasis (PubMed, 24656726). Pharmacological targeting of the matrix involves the use of barrier repair emollients that replenish essential lipids or humectants that enhance the water-holding capacity of the proteinaceous corneocytes. Additionally, keratolytic agents are used to modulate the desquamation process by disrupting the cohesive forces within the matrix to treat hyperkeratotic disorders (StatPearls, NBK545242).
