The lipid component, primarily composed of ceramides, cholesterol, and free fatty acids, forms lamellar sheets that regulate permeability, while the intracellular natural moisturizing factors (NMF) within corneocytes maintain hydration [PubMed: 12553851]. In diseases such as atopic dermatitis and psoriasis, the matrix is often compromised, leading to barrier dysfunction and inflammation [PubMed: 26844894]. Therapeutic interventions target this matrix through the application of emollients, humectants, and barrier-repair lipids to restore structural integrity and improve skin health [PubMed: 30047248]. Additionally, the matrix serves as the rate-limiting step for the percutaneous absorption of topically applied drugs, making it a focal point for the development of chemical penetration enhancers [PubMed: 15003371].
