Overview

Streptococcus pneumoniae capsular polysaccharide serotype 23B is a complex carbohydrate polymer that constitutes the outermost protective layer of the 23B serotype of the pneumococcus bacterium. This polysaccharide is a critical virulence factor that enables the pathogen to evade the host immune system by preventing opsonophagocytosis by leukocytes (Source: PubMed, PMID: 31510102). Serotype 23B has gained clinical prominence as a leading cause of invasive pneumococcal disease (IPD) and antibiotic resistance in the era following the introduction of earlier-generation conjugate vaccines that did not cover this specific serotype (Source: CDC, Pink Book). In pharmaceutical development, this polysaccharide is utilized as a purified antigen in next-generation pneumococcal conjugate vaccines (PCVs), such as the 21-valent vaccine Capvaxive. When conjugated to a carrier protein like CRM197, the polysaccharide induces a T-cell dependent immune response, resulting in the production of high-affinity antibodies and the establishment of immunological memory (Source: FDA, Capvaxive Prescribing Information). These vaccine-induced antibodies bind to the 23B capsule, allowing for complement activation and subsequent destruction of the bacteria, thereby protecting against pneumonia, meningitis, and sepsis.
