Streptococcus pneumoniae capsular polysaccharide serotype 6B (CPS6B) is a complex carbohydrate polymer that constitutes the outermost protective layer of the bacterium. It functions as a primary virulence factor by shielding the pathogen from the host's immune system, specifically by preventing opsonophagocytosis by white blood cells (NIH, 2023). Serotype 6B is a member of Serogroup 6, which includes structurally similar serotypes 6A, 6C, and 6D; these variants differ primarily in the specific linkages between their sugar subunits, such as rhamnose and ribitol (Journal of Clinical Microbiology, 2007). In clinical practice, this polysaccharide is a key component of pneumococcal conjugate and polysaccharide vaccines, where it acts as an antigen to stimulate the production of protective antibodies (CDC, 2022). While antibodies generated against 6B can provide cross-reactivity with 6A, 6C, and 6D, modern multivalent vaccines often include both 6A and 6B to ensure broader coverage across the serogroup (Clinical and Vaccine Immunology, 2011). Immunization targeting this molecule is essential for preventing invasive pneumococcal diseases, including pneumonia, meningitis, and febrile bacteremia, particularly in high-risk populations like children and the elderly (WHO, 2019). The polysaccharide is often conjugated to a carrier protein, such as CRM197, to convert the T-cell independent response into a T-cell dependent one, enhancing memory and efficacy in infants (StatPearls, 2023).
