Overview

The subgingival microbiome is a complex ecological community of microorganisms, including bacteria, archaea, and viruses, that resides within the gingival sulcus or periodontal pocket (StatPearls, 2023). It functions as a structured biofilm where interspecies interactions and metabolic activities maintain a specific niche environment (PubMed, 2021). In a healthy state, the microbiome is symbiotic, but a shift toward dysbiosis—characterized by the proliferation of anaerobic pathogens like Porphyromonas gingivalis—is the primary driver of periodontal diseases such as gingivitis and periodontitis (NIH, 2022). This ecosystem also serves as a source of systemic inflammation, with links to cardiovascular disease and diabetes through the translocation of bacteria and their products into the bloodstream (Journal of Oral Microbiology, 2020). Pharmacological management typically involves the use of antiseptics like chlorhexidine or antibiotics like metronidazole and doxycycline to reduce the pathogen load and restore a health-associated microbial balance (PubMed, 2019). Because it is a multi-species community rather than a single molecular entity, it presents significant challenges for targeted drug delivery and the prevention of antibiotic resistance.

Mechanism of action

Antimicrobial agents target the subgingival microbiome by disrupting bacterial cell walls, inhibiting protein synthesis, or causing DNA damage in anaerobic species, thereby reducing the total microbial load and disrupting the pathogenic biofilm (StatPearls, 2023).
