Overview

The superior medial genicular nerve is a sensory peripheral nerve that originates primarily from the tibial division of the sciatic nerve. It provides essential sensory innervation to the superomedial aspect of the knee joint capsule, including the ligaments and synovial membranes. Clinically, this nerve is not a molecular target like a receptor or enzyme but is a significant anatomical target for interventional pain management procedures such as genicular nerve blocks and radiofrequency ablation (RFA). These procedures are commonly employed to manage chronic knee pain associated with osteoarthritis or to provide analgesia following total knee arthroplasty. By applying local anesthetics to the nerve, clinicians can temporarily interrupt nociceptive signaling, whereas thermal ablation can provide longer-term relief by disrupting the nerve's ability to transmit pain signals. Its precise anatomical course near the medial femoral epicondyle and its proximity to the superior medial genicular artery are critical considerations for safe procedural targeting.
