Overview

Suprabasin (SBSN) is a secreted protein highly conserved across species, originally identified as a component of the cornified envelope in stratified epithelia such as the skin, tongue, esophagus, and forestomach[1][2]. It is involved in epidermal differentiation, keratinization, and maintenance of barrier function, and regulates gene expression through epigenetic mechanisms (e.g., methylation, histone modification)[1]. Its aberrant expression is associated with therapy resistance, cancer progression, and poor prognosis in various malignancies, and its deficiency is linked to altered skin barrier and increased sensitivity to allergens (e.g., nickel, relevant to atopic dermatitis)[2][3]. There are multiple isoforms, but SBSN-1 is the primary form with a unique alanine/glycine/histidine-rich domain. The molecular structure is mostly disordered; its precise physiological mechanisms and therapeutic targeting remain active fields of investigation[1][3][4].

Mechanism of action

Drugs such as 5-azacytidine can induce SBSN expression in cancer cells via epigenetic modification (DNA hypomethylation); suprabasin is implicated in adaptation to therapeutic stress, migration, invasion, and angiogenesis through pathways such as MEK/ERK and NF-κB
