Synovial fluid is a specialized, viscous biological fluid found within the joint space of diarthrodial joints, primarily composed of an ultrafiltrate of plasma and high concentrations of hyaluronan and lubricin (StatPearls, 2023). Its primary biological functions include providing lubrication to minimize friction between articular surfaces, acting as a shock absorber during mechanical loading, and facilitating the transport of nutrients and waste for the avascular articular cartilage (Arthritis Research & Therapy, 2007). While the joint space is an anatomical compartment rather than a specific molecular target, it serves as a critical site for local drug delivery, known as intra-articular injection, to treat various joint pathologies (Journal of Clinical Medicine, 2021). In diseases such as osteoarthritis and rheumatoid arthritis, the composition of the synovial fluid is altered, often containing elevated levels of inflammatory cytokines and degradative enzymes that contribute to joint destruction (Nature Reviews Rheumatology, 2017). Therapeutic interventions targeting this space aim to restore joint homeostasis, reduce inflammation, and provide symptomatic relief through the administration of viscosupplements or anti-inflammatory agents (Annals of the Rheumatic Diseases, 2020).
Intra-articular administration provides localized therapeutic effects by delivering high concentrations of drugs directly to the joint tissues, thereby reducing systemic side effects and targeting the synovial membrane and cartilage (StatPearls, 2023).
