The synovial fluid extracellular matrix and hyaluronan-receptor milieu represent the complex physiological environment within the joint capsule that maintains articular health. This milieu is primarily composed of high-molecular-weight hyaluronan (HA), lubricin, and various proteins that provide essential lubrication and shock absorption for cartilage (Tamer, 2013). In pathological states such as osteoarthritis, the degradation of HA by hyaluronidases and reactive oxygen species leads to a loss of viscoelasticity and increased friction (Gupta et al., 2019). Furthermore, the interaction between HA and its primary cell-surface receptor, CD44, regulates inflammatory signaling and chondrocyte metabolism (Misra et al., 2015). Therapeutic strategies targeting this non-specific milieu, such as viscosupplementation, involve the intra-articular injection of exogenous HA to restore rheological properties (Moreland, 2003). These treatments aim to reduce pain and improve joint mobility by mimicking the physical properties of healthy synovial fluid. Additionally, exogenous HA may exert anti-inflammatory effects by saturating HA receptors and inhibiting the production of pro-inflammatory cytokines. This target is considered non-specific because it encompasses a broad range of molecular interactions and physical properties rather than a single protein or pathway.
