Overview

The Synovial sarcoma X breakpoint 2 (SSX2) peptide–MHC class I complex is a tumor-specific antigen presentation assembly consisting of a degraded fragment of the SSX2 protein bound to a Major Histocompatibility Complex (MHC) class I molecule, most commonly HLA-A*02:01 (UniProt: Q16385) [1]. SSX2 belongs to the cancer-testis antigen (CTA) family, which is typically expressed only in the immune-privileged environment of the testis but becomes aberrantly re-expressed in various cancers, including synovial sarcoma, melanoma, and multiple myeloma (PMID: 12130514) [2]. This restricted expression pattern makes the SSX2-MHC complex a highly attractive target for immunotherapy, as it minimizes the risk of attacking healthy somatic tissues. Therapeutic strategies primarily involve T-cell receptor (TCR) engineered T-cell therapies, such as GSK3845097, which are designed to recognize and bind this specific peptide-MHC complex on the surface of malignant cells (NCT04843917) [3]. Upon binding, these engineered T cells initiate a potent cytotoxic response, leading to the targeted destruction of the tumor. However, the development of these therapies requires rigorous screening to prevent off-target cross-reactivity with similar peptides found in vital organs, which could lead to severe adverse events.
