Syntaxin-binding protein 5-like (STXBP5L) is a cytosolic protein that participates in the regulation of vesicle trafficking and exocytosis inhibition. In pancreatic beta-cells, it suppresses insulin secretion by interacting with and regulating syntaxin 1A (STX1A) and syntaxin 4 (STX4), both of which are crucial t-SNARE proteins for insulin granule-plasma membrane fusion. In neurons, this protein blocks neurotransmitter (e.g., acetylcholine) release from axon terminals and helps prevent synaptic fatigue under repetitive stimulation. It also plays a role in promoting axonal outgrowth. Pathogenic variants in the STXBP5L gene have been associated with infantile-onset neurodegenerative disorders, autism spectrum disorder, and craniosynostosis. There are currently no drugs known to target this protein directly, nor established use as a biomarker or clear safety concerns described in the literature[1][4][7][11].
