Systemic acid–base balance via sodium lactate–bicarbonate buffering is a physiological mechanism essential for maintaining the body's pH within the homeostatic range of 7.35 to 7.45. This process is primarily mediated by the conversion of lactate into bicarbonate, which serves as the principal extracellular buffer against metabolic acidosis (StatPearls, 2023). When sodium lactate is administered, it is transported into hepatocytes where it undergoes oxidative metabolism or gluconeogenesis, effectively consuming hydrogen ions and increasing systemic bicarbonate levels (PubChem, 2024). This buffering system is a cornerstone of clinical fluid therapy, particularly in the form of Ringer's lactate solution, used to treat volume depletion and metabolic imbalances (Merck Manual, 2023). However, the efficacy of this mechanism is highly dependent on hepatic and renal function, as impaired metabolism can lead to lactate accumulation rather than pH correction. Clinically, this system is monitored through blood gas analysis, focusing on pH, bicarbonate concentration, and base excess to ensure therapeutic efficacy and avoid complications like metabolic alkalosis or hypernatremia.
Sodium lactate acts as an alkalizing agent by being metabolized, primarily in the liver, into bicarbonate. This metabolic process consumes hydrogen ions or produces bicarbonate ions, which then circulate systemically to neutralize excess acidity and restore physiological pH levels.
