Overview

Systemic and tissue cholesterol pools represent the total distribution of cholesterol throughout the body, encompassing both plasma lipoproteins and cellular membranes. Cholesterol is an essential lipid required for maintaining cell membrane fluidity and integrity (StatPearls, 2023). It serves as the primary precursor for the synthesis of steroid hormones, vitamin D, and bile acids necessary for lipid digestion (Nature Reviews Cardiology, 2017). The balance of these pools is maintained through a complex interplay of endogenous synthesis, dietary absorption, and biliary excretion. Dysregulation of these pools, particularly an excess of systemic low-density lipoprotein cholesterol (LDL-C), is a primary driver of atherosclerosis and cardiovascular disease (NIH, 2022). Pharmacological interventions target these pools by inhibiting key enzymes like HMG-CoA reductase or blocking intestinal transporters like NPC1L1. Other therapies enhance the recycling of LDL receptors to promote clearance from the systemic circulation. Managing these pools is critical for reducing the risk of myocardial infarction and stroke.

Mechanism of action

Drugs modulate these pools by inhibiting HMG-CoA reductase to reduce synthesis, blocking NPC1L1 to reduce intestinal absorption, or inhibiting PCSK9 to increase hepatic LDL receptor expression and plasma clearance (Nature Reviews Cardiology, 2017).
