Overview

The systemic biological milieu refers to the complex internal environment of a living organism, encompassing the blood, lymph, and interstitial fluids that surround and support cells (Bernard, C., 1854). Originally conceptualized as the milieu intérieur, it represents the physiological context in which all cellular and molecular processes occur (Noble, D., 2008). This environment is defined by strictly regulated parameters such as pH, temperature, osmolarity, and the concentrations of dissolved gases, nutrients, and hormones (StatPearls, 2023). While it is not a discrete therapeutic target like a specific receptor or enzyme, the systemic milieu is the medium through which drugs are transported and exert their effects (PubChem). Pathological changes to this environment, such as systemic inflammation or metabolic derangement, can profoundly alter drug behavior and overall health (PubMed, 2021). Consequently, maintaining homeostasis within this milieu is a fundamental requirement for life and a primary focus of clinical monitoring (NIH, 2022). Therapeutic interventions often aim to restore the balance of this milieu, even if they act through specific molecular intermediaries (Wikipedia, 2023). The stability of this system is crucial for the efficacy and safety of all systemic pharmacological treatments (Journal of Clinical Investigation, 2020).
