Overview

Systemic calcium balance is the physiological process of maintaining extracellular calcium levels within a narrow range, which is critical for skeletal integrity, nerve conduction, and muscle function [1]. This homeostatic system is regulated by a complex feedback loop involving the parathyroid glands, kidneys, bone, and intestines, primarily mediated by parathyroid hormone (PTH) and vitamin D [2]. The calcium-sensing receptor (CaSR) serves as the primary molecular sensor, detecting changes in blood calcium and adjusting PTH secretion to maintain equilibrium [3]. Dysregulation of this system leads to significant morbidity, including osteoporosis, hyperparathyroidism, and mineral-bone disorders associated with chronic kidney disease [4]. Therapeutic strategies target various nodes of this system, such as using calcimimetics to modulate CaSR activity or antiresorptive agents to influence bone calcium release [1,4]. Understanding the systemic interplay of these components is essential for developing treatments for metabolic bone diseases and electrolyte imbalances.

Mechanism of action

Drugs modulate systemic calcium balance by targeting specific regulatory components: calcimimetics activate the calcium-sensing receptor (CaSR) to lower PTH; bisphosphonates and RANKL inhibitors reduce bone resorption; and PTH analogs stimulate osteoblastic activity to increase bone mass [1,4].
