Systemic cellular environment

The systemic cellular environment, historically termed the milieu intérieur, represents the complex and integrated extracellular landscape that surrounds and supports the cells of a multicellular organism (Wikipedia, 2023). It is not a single molecular target such as a receptor or enzyme, but rather a physiological context comprising the interstitial fluid, plasma, signaling molecules, and the extracellular matrix that collectively maintain homeostasis (NIH, 2022). In various pathologies, the systemic environment undergoes significant alterations; for instance, in cancer, the systemic milieu can be co-opted to support tumor progression through chronic inflammation and metabolic reprogramming (Nature Reviews Cancer, 2020). While pharmacological agents are designed to interact with specific molecular components within this environment, the overall state of the systemic environment dictates drug bioavailability, efficacy, and the profile of adverse effects (PubMed, 2021).
