Overview

The systemic extracellular fluid (ECF) and electrolyte compartments represent the physiological spaces outside of cells, primarily composed of interstitial fluid and blood plasma (StatPearls, 2023). These compartments serve as the primary medium for the transport of nutrients, oxygen, and waste products between the vascular system and the cells (NCBI, 2022). Electrolytes within these spaces, most notably sodium, chloride, and bicarbonate, are essential for maintaining osmotic pressure, acid-base balance, and membrane potentials. While not a discrete molecular target like a protein or receptor, these compartments are the focus of intensive clinical management through the use of intravenous fluids and diuretics (Merck Manual, 2023). Drugs like loop diuretics or thiazides act on specific transporters within the renal tubules to influence the volume and composition of the ECF. Disorders of these compartments, such as hyponatremia, hyperkalemia, or fluid overload, are common in clinical practice and can lead to severe complications including cardiac arrhythmias or neurological impairment.

Mechanism of action

Pharmacological agents modulate these compartments by inhibiting ion transporters (e.g., NKCC2, NCC), creating osmotic gradients, or directly supplementing fluid and electrolyte deficits (StatPearls, 2023).
