The systemic glucose pool refers to the total mass of glucose present within the extracellular fluid, including the blood plasma and interstitial fluid. It serves as the essential substrate for ATP production across all human tissues and is the exclusive energy source for the brain under non-starvation conditions (StatPearls, 2023). Regulation of this pool is achieved through the coordinated actions of the endocrine system, where insulin promotes glucose clearance and glucagon stimulates glucose release from the liver (NIH, 2022). In clinical practice, the systemic glucose pool is not a discrete molecular target but rather the physiological compartment that drugs for diabetes aim to stabilize. Dysregulation of this pool leads to pathological states such as diabetes mellitus, where chronic elevation (hyperglycemia) causes microvascular and macrovascular damage. Conversely, acute depletion of the pool results in hypoglycemia, which can lead to immediate neurological impairment or death (PubMed, 2021). Pharmacological interventions, such as insulin sensitizers or secretagogues, work by altering the rates of glucose entry into or exit from this pool. Therefore, the systemic glucose pool represents the primary endpoint for efficacy in metabolic drug development.
Drugs modulate the systemic glucose pool by altering the rates of glucose appearance (via hepatic gluconeogenesis or intestinal absorption) and disappearance (via tissue uptake or renal excretion).
