The systemic hemostatic machinery is a highly regulated physiological system responsible for maintaining vascular integrity by preventing blood loss following injury while ensuring blood remains fluid under normal conditions (StatPearls, 2023). This system comprises three primary components: primary hemostasis involving platelet plug formation, secondary hemostasis consisting of the coagulation cascade resulting in fibrin formation, and fibrinolysis which involves the breakdown of clots (NIH, 2022). It involves a complex interplay of vascular endothelial cells, platelets, and numerous plasma proteins, including clotting factors and their inhibitors (Merck Manual, 2023). Dysfunction in this machinery can lead to life-threatening conditions such as deep vein thrombosis, pulmonary embolism, or hemophilia. Therapeutic strategies often target specific molecular components within this system, such as Thrombin or Factor Xa, to manage thrombotic risks or treat bleeding disorders (PubMed, 2021).
Drugs targeting this system act via various mechanisms including inhibition of coagulation factors (e.g., Factor Xa or Thrombin), inhibition of platelet aggregation, or activation of the fibrinolytic pathway to dissolve existing clots (StatPearls, 2023; NIH, 2022).
