Overview

T-box transcription factor 18 (TBX18) is a DNA-binding transcription factor from the T-box family, essential for embryonic development of the vertebral column, ureter, posterior heart, and specification of mesenchymal and smooth muscle cell lineages. TBX18 functions mainly as a transcriptional repressor, forming homo- and heterodimers with other transcription factors to regulate gene expression. Mutations or dysregulation of TBX18 are linked to congenital anomalies such as CAKUT and heart defects. In cardiac cells, TBX18 suppresses Cx43 to confer pacemaker cell properties, and overexpression can reprogram cells into pacemaker-like phenotypes—a strategy under preclinical development using modified mRNA. TBX18 interacts with multiple cofactors and homeobox proteins, indicating complex regulatory networks in development and disease contexts.
