Overview

The T-cell receptor (TCR) recognition of peptide–MHC class II (pMHC-II) complexes is the fundamental molecular interaction that triggers the adaptive immune response in CD4+ T helper cells [1]. This process occurs when the TCR, a surface-bound heterodimer, binds to a specific peptide antigen presented within the groove of an MHC class II molecule (such as HLA-DR, HLA-DQ, or HLA-DP) on an antigen-presenting cell [2]. This recognition event is stabilized by the CD4 co-receptor and leads to the formation of an immunological synapse, initiating intracellular signaling cascades that drive T-cell differentiation and cytokine production [1, 5]. In autoimmune diseases, this system fails as TCRs recognize self-peptides, leading to tissue destruction, whereas in cancer, the interaction may be evaded by tumors to avoid immune detection [2]. Therapeutic interventions targeting this complex include immunosuppressants that block TCR signaling, peptide analogs like glatiramer acetate that compete for MHC binding, and engineered TCR-T cell therapies designed to redirect the immune system against specific pathogens or malignant cells [3, 5].
