The T-cell receptor (TCR) on CD4+ T cells that recognizes peptides derived from the HIV-1 Nef (Negative Factor) protein presented by Human Leukocyte Antigen (HLA) class II molecules is a critical component of the adaptive immune response against HIV-1 (PMID: 17914080). Nef is an early-expressed accessory protein essential for viral replication, pathogenesis, and immune evasion, making it a primary target for therapeutic intervention (PMID: 2017396). These specific TCRs enable CD4+ T cells to identify HIV-infected cells or antigen-presenting cells that have processed viral proteins, coordinating a broad antiviral response (PMID: 30104374). In the field of HIV cure research, these TCRs are being isolated and utilized to develop TCR-engineered T-cell therapies (TCR-T) designed to enhance the clearance of the latent HIV reservoir (PMID: 33619386). By recognizing Nef-derived epitopes, these engineered cells can trigger cytokine release and direct cytolytic activity against infected cells, potentially overcoming the limitations of the natural immune response. The interaction between the TCR and the Nef-HLA complex is highly specific, and its efficacy depends on the patient's HLA genotype and the conservation of the targeted Nef epitope. Therapeutic strategies targeting this receptor aim to restore or augment the T-cell-mediated control of HIV-1 in the absence of antiretroviral therapy. However, challenges such as viral mutational escape and potential off-target cross-reactivity with host proteins remain significant hurdles in clinical development (PMID: 28254817).
