Overview

T cell receptors (TCRs) recognizing melanoma peptide-MHC complexes are specialized immune receptors engineered to target intracellular melanoma-associated antigens (MAAs) presented on the cell surface. Unlike CAR-T cells that recognize surface antigens, these TCRs identify short peptide fragments—such as those from MART-1, gp100, NY-ESO-1, or MAGE-A4—bound to Major Histocompatibility Complex (MHC) molecules, most commonly HLA-A*02:01 (PMID: 19171878, 21282551). This recognition mechanism allows the therapeutic targeting of a vast array of internal proteins that are otherwise inaccessible to traditional antibody-based therapies. Current therapeutic modalities include TCR-engineered T-cell (TCR-T) therapies and bispecific T-cell engagers (ImmTACs) like Tebentafusp, which redirect the patient's immune system to eliminate tumor cells (PMID: 34551229). Upon binding the specific peptide-MHC complex, the TCR initiates a signaling cascade through the CD3 complex, resulting in T-cell proliferation and the release of cytotoxic molecules like granzymes and perforins. However, clinical application is often complicated by on-target, off-tumor toxicities, as many targeted antigens are also expressed in healthy melanocytes found in the skin, eyes, and inner ear (PMID: 19171878).
