T-cell surface glycoprotein CD1 (CD1) is a family of highly conserved, Major Histocompatibility Complex (MHC) class I-like glycoproteins specialized in the presentation of lipid and glycolipid antigens to T cells. Unlike classical MHC molecules that present peptide fragments, CD1 proteins possess a deep, hydrophobic antigen-binding groove adapted to accommodate aliphatic hydrocarbon chains [1, 4]. In humans, the family comprises five isoforms categorized into three groups: Group 1 (CD1a, CD1b, CD1c), Group 2 (CD1d), and Group 3 (CD1e), with CD1d being the most extensively studied therapeutic target due to its role in activating Natural Killer T (NKT) cells [3, 10, 16]. Therapeutically, CD1d agonists such as Alpha-galactosylceramide are utilized to stimulate NKT cells, bridging innate and adaptive immunity to enhance anti-tumor responses or treat chronic infections like tuberculosis [8, 9, 13]. While CD1a-c are primarily expressed on specialized antigen-presenting cells like dendritic cells, CD1d is expressed more broadly across hematopoietic and some non-hematopoietic tissues, making it a versatile target for immunotherapy [4, 5, 17]. Ongoing research focuses on modulating these pathways to treat cancer, autoimmune disorders, and allergic conditions by fine-tuning the balance of pro-inflammatory and anti-inflammatory cytokine release [1, 14, 21].
CD1 molecules (primarily CD1d) function as antigen-presenting platforms that bind lipid and glycolipid antigens in a deep hydrophobic groove. Agonists such as Alpha-galactosylceramide bind to CD1d, forming a complex recognized by the semi-invariant T-cell receptor (TCR) of Natural Killer T (NKT) cells. This recognition triggers rapid activation of NKT cells, leading to the secretion of a broad range of cytokines (e.g., IFN-gamma, IL-4) that modulate both innate and adaptive immune responses [4, 6, 11].
