Overview

The "T helper type 1 cell / T helper type 2 cell balance" refers to the relative functional activity between two major subtypes of CD4+ T helper lymphocytes: Th1 and Th2 cells. Th1 cells drive cellular immune responses against intracellular pathogens via secretion of IFNγ and related cytokines, while Th2 cells promote humoral (antibody-mediated) responses and protection against extracellular pathogens by releasing cytokines such as IL-4, IL-5, and IL-13. This balance is orchestrated by external cytokine milieu, master transcription factors (T-bet for Th1, GATA3 for Th2), and antigen exposure. Imbalance can lead to disease: Th1 dominance is linked to autoimmunity and chronic inflammation, while Th2 dominance is associated with allergic diseases and IgE-mediated conditions. Therapies seek to shift this balance in diseases of Th1 or Th2 dominance but must be used carefully due to the risk of tipping immune responses towards immunodeficiency or hypersensitivity.
