Overview

TBX5 antisense RNA 1 (TBX5-AS1) is a long non-coding RNA classified as both an antisense RNA and an enhancer RNA, transcribed from the TBX5 genomic locus. It is non-protein coding and modulates expression of the TBX5 gene, which encodes a T-box transcription factor critical for heart development and tumor suppression. TBX5-AS1 is mainly nuclear and can form RNA–RNA duplexes with the sense TBX5 transcript, thereby affecting TBX5 mRNA stability and expression. Its downregulation has been implicated in various cancers and congenital heart diseases: in cancer, TBX5-AS1 acts as a tumor suppressor by inactivating the PI3K/AKT pathway, thereby inhibiting cell proliferation, migration, and invasion, and promoting apoptosis. In Tetralogy of Fallot—a congenital heart defect—hypermethylation of TBX5-AS1 leads to decreased TBX5 expression and impaired proliferation. TBX5-AS1 is under investigation as a diagnostic and prognostic biomarker and potentially as a therapeutic target, though no drugs directly target it yet and mechanistic details remain to be clarified.
