Overview

The tear film stabilization/moisturization system refers collectively to the mechanisms that maintain moisture on the ocular surface. It involves three main layers—lipid, aqueous, and mucinous—that work together for lubrication, protection against desiccation and infection, maintenance of optical clarity for vision quality, removal of debris, as well as supporting corneal epithelial health. Key components include lipids from meibomian glands forming an outer hydrophobic layer that prevents evaporation; an aqueous layer produced by lacrimal glands providing hydration; and an inner mucous layer secreted by conjunctival goblet cells that facilitates even spreading over the cornea. Proteins such as lactoferrin, lipocalin, immunoglobulins like sIgA, lysozyme, β-lysin, contribute antimicrobial activity while also stabilizing the structure. Disruption leads to dry eye disease characterized by discomfort visual disturbance due increased friction between eyelids cornea during blinking.

Mechanism of action

Artificial tears/lubricants: Replace deficient aqueous layer and provide lubrication. Mucin secretagogues/anti-inflammatory agents: Stimulate mucin production or reduce inflammation to restore tear film stability. Nerve growth factor-based therapies: Promote epithelial proliferation/differentiation and stimulate mucin production by goblet cells.
