Overview

Technetium-99m (99mTc) is a metastable nuclear isomer of technetium-99 and is the most widely used radioisotope in diagnostic nuclear medicine (StatPearls, 2023). It functions as a gamma-emitting tracer that enables non-invasive visualization of physiological processes via Single Photon Emission Computed Tomography (SPECT) (PubChem, 2024). As a metal center, it is typically coordinated with various organic ligands to form radiopharmaceuticals that localize to specific organs or tissues based on the ligand's biological properties (NIH NCI, 2024). These complexes are utilized to diagnose a variety of conditions, including myocardial ischemia, bone metastases, and renal dysfunction (Wikipedia, 2024). Unlike traditional therapeutic targets such as receptors or enzymes, the 99mTc metal center serves as a reporter molecule rather than a site of pharmacological action. Its favorable physical properties, including a 6-hour half-life and a 140 keV photon energy, provide an optimal balance between high-quality imaging and minimized patient radiation dose (StatPearls, 2023). The coordination chemistry of the technetium metal center allows for the design of diverse imaging agents tailored to specific physiological pathways.

Mechanism of action

Technetium-99m decays via isomeric transition, emitting a 140 keV gamma photon that is detected by gamma cameras for Single Photon Emission Computed Tomography (SPECT) imaging (StatPearls, 2023).
