Overview

Testosterone production is a tightly-regulated physiological process central to male sexual differentiation, fertility, and anabolic functions. The process is primarily governed by the hypothalamic-pituitary-gonadal axis: gonadotropin-releasing hormone (GnRH) from the hypothalamus stimulates the pituitary to release luteinizing hormone (LH), which acts on Leydig cells in the testes to convert cholesterol into testosterone via a sequence of enzymatic reactions. Testosterone is vital for developing male reproductive organs, secondary sex characteristics, muscle and bone mass, and maintaining libido. Dysregulation of its production can result in a range of clinical conditions affecting reproductive, metabolic, and psychological health[1][3][4][5][6]. Testosterone production does not refer to a single molecule or receptor, so it is not a direct therapeutic target but rather a pathway involving several molecular targets (e.g., LH receptor, steroidogenic enzymes), making its designation as a "target" incorrect under standard conventions.
