Overview

The testosterone synthesis and release pathway is a multi-step endocrine process primarily regulated by the Hypothalamic-Pituitary-Gonadal (HPG) axis (StatPearls, 2023). It begins with the pulsatile secretion of Gonadotropin-Releasing Hormone (GnRH) from the hypothalamus, which stimulates the anterior pituitary to release Luteinizing Hormone (LH) and Follicle-Stimulating Hormone (FSH) (NIH, 2022). LH then binds to its G protein-coupled receptor on testicular Leydig cells, initiating a steroidogenic cascade where cholesterol is converted into testosterone through enzymes like CYP11A1 and CYP17A1 (UniProt, 2024). This pathway is critical for the development of male secondary sexual characteristics, bone density, and spermatogenesis. In clinical practice, this pathway is a major therapeutic target; for instance, androgen deprivation therapy (ADT) using GnRH agonists or CYP17A1 inhibitors is a standard treatment for advanced prostate cancer to reduce circulating testosterone levels (PubMed, 2021). Conversely, components of the pathway are targeted with replacement therapies or gonadotropins to treat male hypogonadism and infertility.
