Thallium is a heavy metal element (atomic number 81) that is not a biological target but rather a potent systemic toxicant and a diagnostic tool in nuclear medicine (StatPearls: NBK507803). It exerts its biological effects primarily by mimicking potassium ions (K+), as it possesses a similar ionic radius, allowing it to be transported into cells via the Na+/K+-ATPase pump and various potassium channels (Wikipedia: Thallium). This mimicry leads to the disruption of essential potassium-dependent processes, including oxidative phosphorylation and protein synthesis, resulting in multi-organ toxicity characterized by alopecia and peripheral neuropathy (PubChem: CID 5359484). In clinical settings, the radioisotope Thallium-201 is utilized as a tracer for myocardial perfusion imaging to assess coronary artery disease and myocardial viability (NIH: Thallium-201). Because it is an element and a toxicant rather than a therapeutic protein or receptor, it is classified as an incorrect target for drug development, though its removal is the goal of specific antidotes like Prussian blue (NIH: Prussian Blue).

Mechanism of action
