Therapeutic cargo refers to the active biological or chemical payload delivered by a carrier system to a specific site of action to achieve a clinical effect. This term is commonly used in the fields of nanomedicine, gene therapy, and targeted drug delivery to distinguish the active agent from its delivery vehicle, such as lipid nanoparticles, viral vectors, or monoclonal antibodies (Mitchell et al., 2021). Examples of therapeutic cargo include nucleic acids like mRNA or siRNA, small molecule chemotherapeutics, and functional proteins or enzymes. Because Therapeutic Cargo is a functional classification rather than a specific molecular target like a receptor or enzyme, it does not have a singular biological role or mechanism. Instead, its function is defined by the specific molecule being transported and the disease context, such as inducing apoptosis in cancer cells or replacing a deficient protein in genetic disorders (Chau et al., 2019). The safety and efficacy of the cargo are often dependent on the efficiency of the delivery system and the specificity of the release mechanism at the target site. Common challenges include avoiding premature degradation in the bloodstream and minimizing off-target toxicity caused by the cargo's inherent biological activity. Consequently, Therapeutic Cargo is not a therapeutic target itself but represents the therapeutic component of a complex drug delivery system.
The mechanism of action is not intrinsic to the term Therapeutic Cargo but is determined by the specific molecular entity being delivered; for instance, siRNA cargo acts via the RNA-induced silencing complex (RISC) to degrade target mRNA, while cytotoxic payloads in antibody-drug conjugates (ADCs) typically act by inhibiting tubulin polymerization or inducing DNA strand breaks (Mitchell et al., 2021; Chau et al., 2019).
