Overview

Thiamine is a water-soluble vitamin essential for carbohydrate metabolism, energy production, and nervous system health. The biologically active form, thiamine pyrophosphate, acts as a coenzyme for key metabolic enzymes. Deficiency leads to diseases such as Beriberi and Wernicke-Korsakoff syndrome. Thiamine is not typically considered a drug target but its derivatives are sometimes applied therapeutically in neuropathy and pain management. Pyridoxine (vitamin B6) is a water-soluble vitamin critical for amino acid metabolism, neurotransmitter synthesis, and hemoglobin production. It is used therapeutically to prevent and treat vitamin B6 deficiency, isoniazid-induced neuropathy, and sometimes in analgesic combinations for neuropathic and back pain. Like thiamine, pyridoxine is not a traditional molecular target but an essential nutrient and enzyme cofactor.

Mechanism of action

Thiamine functions as thiamine pyrophosphate, a coenzyme for multiple enzymes in glucose and amino acid metabolism, promoting conversion of carbohydrates into energy. It may also modulate voltage-gated sodium channels and have anti-inflammatory/analgesic effects—possible mechanisms in pain management when combined with NSAIDs. Pyridoxine functions as a coenzyme (primarily as pyridoxal phosphate) in amino acid, glucose, and lipid metabolism. In pain management, it may block P2X receptors and modulate GABAergic and serotoninergic neurotransmission.
