Phl p 4 is a major 55-60 kDa glycoprotein allergen derived from Timothy grass (Phleum pratense) pollen. Biologically, it is a member of the berberine bridge enzyme-like (BBE-like) family and functions as a glucose dehydrogenase with a bicovalently attached flavin adenine dinucleotide (FAD) cofactor. In the context of human disease, Phl p 4 is a primary driver of seasonal allergic rhinitis and asthma, with IgE-binding reactivity observed in approximately 75-82% of grass-pollen-allergic individuals. In the biotechnology and pharmaceutical sectors, Phl p 4 serves as a critical component in component-resolved diagnosis (CRD) to differentiate genuine grass sensitization from cross-reactivity caused by other plant species. It is a major target for allergen-specific immunotherapy (AIT), where both native extracts and recombinant versions are utilized to desensitize patients. Clinical studies highlight Phl p 4 as an early indicator of 'molecular spreading' in children, where sensitization to this molecule often predicts the future development of symptomatic hay fever. Its heavy glycosylation with cross-reactive carbohydrate determinants (CCDs) presents a significant challenge in diagnostic specificity, as these chains can bind IgE from patients sensitized to unrelated plant species without necessarily causing clinical symptoms.

Mechanism of action

Allergen-specific immunotherapy (AIT) works by inducing immune tolerance through the repeated administration of the allergen, which triggers a shift from a Th2-mediated allergic response to a Th1/Treg-mediated regulatory response and stimulates the production of allergen-specific blocking IgG4 antibodies.
