Tissue factor pathway inhibitor 1 (TFPI1), often abbreviated as TFP1 in clinical literature, is a primary physiological regulator of the extrinsic pathway of blood coagulation. It functions as a multi-valent Kunitz-type serine protease inhibitor that directly neutralizes Factor Xa and, in a Factor Xa-dependent manner, inhibits the Factor VIIa-tissue factor complex. Beyond its role in hemostasis, TFPI1 is involved in modulating inflammatory responses and has been implicated in the progression of various cancers and cardiovascular diseases. In therapeutic development, TFPI1 is a major target for treating hemophilia, where monoclonal antibodies like concizumab and marstacimab are used to inhibit TFPI1 to promote thrombin generation. Conversely, its role in preventing microvascular thrombosis makes it a subject of study in sepsis and acute coronary syndromes.
