Overview

The Toll-like receptor 4 (TLR4) complex is a multi-protein assembly essential for the innate immune system's ability to detect Gram-negative bacteria (Wikipedia, 2024). The core of this complex is a heterodimer consisting of Toll-like receptor 4 and the accessory protein MD-2, also known as Lymphocyte antigen 96 (UniProt, 2024). Recognition of the bacterial endotoxin lipopolysaccharide (LPS) requires the coordinated action of LPS-binding protein (LBP) and CD14, which facilitate the transfer of LPS to the TLR4/MD-2 signaling unit (NIH, 2023). Upon activation, the complex undergoes dimerization and initiates intracellular signaling cascades, primarily through the MyD88 and TRIF pathways, leading to the expression of pro-inflammatory cytokines such as TNF-alpha and IL-6 (StatPearls, 2023). While vital for pathogen clearance, overactivation of the TLR4 complex is implicated in the pathogenesis of sepsis, chronic inflammatory diseases, and certain cancers (MDPI, 2022). Therapeutic strategies targeting this complex include antagonists like Eritoran to mitigate excessive inflammation and agonists like Monophosphoryl lipid A used as potent vaccine adjuvants (PubMed, 2022).
