Tooth enamel porosity refers to the presence of minute spaces or pores within the highly mineralized enamel tissue covering the tooth crown. Enamel is composed chiefly of tightly packed hydroxyapatite crystals (95–98% inorganic by weight), arranged in parallel rods with surrounding organic matrix and water[1][6][2][4]. Despite being the hardest tissue in the human body and highly resistant to wear, enamel is not a solid, impervious substance. Small pores contribute to enamel’s limited permeability, allowing diffusion of ions and fluids between the tooth surface and deeper layers, which is critical for processes such as remineralization and demineralization[1][2][3]. Enamel porosity is lowest at the outer surface and slightly higher near the dentinoenamel junction, varying with age, crystal arrangement, and ionic composition[2][1]. Elevated porosity can lead to areas of weakness susceptible to acid attack, resulting in demineralization, white spot lesions, and dental caries[1][4]. Enamel porosity is a material property and not a molecular entity or therapeutic target.
