These secreted proteins are critical regulators of systemic homeostasis, influencing erythropoiesis, skeletal muscle growth, and vascular remodeling by signaling through Activin type II receptors (ActRIIA/B) and subsequent Smad transcription factors (UniProt P08476, O14793, O95390). In diseases such as pulmonary arterial hypertension (PAH), an imbalance occurs where overactive Activin and GDF signaling (Smad2/3) promotes pathological vascular remodeling, while BMP-mediated signaling (Smad1/5/8) is diminished (Hoeper et al., 2023, NEJM). Therapeutic strategies utilize 'ligand traps' like Sotatercept and Luspatercept, which are fusion proteins that sequester these circulating ligands to prevent receptor activation and restore signaling equilibrium (FDA, Winrevair Label; FDA, Reblozyl Label). By neutralizing these specific ligands, these drugs can improve red blood cell maturation in anemias or reverse vascular thickening in PAH. Clinical management of these targets requires careful monitoring of hematological parameters due to their potent effects on red blood cell production.

Mechanism of action
