Transmembrane emp24 domain-containing protein 5 (TMED5) is a member of the p24 protein family involved in the early secretory pathway within cells. TMED5 is an integral membrane protein located mainly in the cis-Golgi network, the endoplasmic reticulum exit sites, and the ER-Golgi intermediate compartment. Structurally, TMED5 contains a GOLD domain (for Golgi dynamics), a coiled-coil domain, a single transmembrane region, and a short cytoplasmic tail that interacts with COPI and COPII coat protein complexes, facilitating the selection of cargo and vesicle formation during protein trafficking. TMED5 is essential for the maintenance of the Golgi apparatus and protein trafficking between the ER and Golgi. Biologically, TMED5 is implicated in cell cycle regulation, apoptosis, and autophagy, through involvement in signaling pathways such as mTOR and TGF-β. In cancer, notably hepatocellular carcinoma and cervical cancer, TMED5 is often overexpressed and associated with worse outcomes and tumor progression; knockdown studies have shown suppressed proliferation, migration, invasion, and enhanced apoptosis. TMED5 is under investigation as a potential cancer therapeutic target and biomarker, but no clinically approved drugs currently target this protein
