Transmembrane protein 18 (TMEM18) is a small, highly conserved transmembrane protein found predominantly at the nuclear membrane, with 3–4 predicted membrane-spanning domains. It harbors a nuclear localization signal and may oligomerize through its transmembrane regions. TMEM18 is implicated in the regulation of neural stem cell migration, possible transcriptional repression via sequence-specific DNA binding, and molecular transport across the nuclear envelope. Genome-wide association studies have robustly linked variants near TMEM18 to obesity and altered metabolic traits in humans, particularly body mass index (BMI) and waist circumference. TMEM18 is expressed in various tissues, notably in the brain, and may act downstream of energy regulatory pathways (insulin/glucagon signaling). The precise molecular function and mechanisms remain incompletely defined, though bioinformatics suggests distant homology to certain ion channel families. No drugs are currently known to target TMEM18, and its direct therapeutic modulation is not established, but its genetic association with obesity makes it a candidate for further investigation as a disease risk gene and possible drug target.
