Overview

The trigeminocervical complex (TCC) is a functional region located in the dorsal horns of the medulla and upper cervical spinal cord (C1–C3). It acts as a key relay site for the convergence of sensory afferents from the cranial (trigeminal) and cervical nerves. Lamina I neurons within this region integrate nociceptive input from both the head (via trigeminal nerve) and neck (via upper cervical nerves), which is then relayed to higher centers in the thalamus and cortex. The TCC is critically involved in the pathogenesis of primary headaches (including migraine), the mechanism of referred craniofacial pain, and the central modulation of cervico-cranial nociception. Drug therapies for migraine—including triptans, CGRP antagonists, NSAIDs, and some nerve blocks—exert their effects in part by modulating neural activity within this complex region
