Overview

Troponin T is one of three subunits comprising the troponin complex found in striated muscles—skeletal and cardiac but not smooth muscle. It binds tropomyosin and helps position it on actin filaments within muscle fibers. This positioning enables regulation of muscular contraction by modulating access to myosin-binding sites on actin in response to changes in intracellular calcium concentration. The gene encoding human cardiac Troponin T is TNNT2; there are also slow skeletal (TNNT1) and fast skeletal isoforms (TNNT3). Cardiac-specific Troponin T released into blood serves as an essential clinical marker for diagnosing acute myocardial infarction and assessing prognosis after various forms of heart damage or stress. Mutations in certain isoforms can lead to inherited myopathies or cardiomyopathies due to altered contractile regulation.
