Overview

The tumor and immune microenvironment (TIME) is a complex, dynamic ecosystem surrounding a tumor, composed of malignant cells, immune cells (such as T cells, macrophages, and myeloid-derived suppressor cells), fibroblasts, endothelial cells, and the extracellular matrix (NCI, 2023). It is characterized by a unique milieu of cytokines, chemokines, and metabolites that often promote tumor growth and facilitate immune evasion (Binnewies et al., Nature Medicine, 2018). While the TIME itself is not a single molecular target, it serves as the critical landscape for various therapeutic interventions, including immune checkpoint inhibitors and anti-angiogenic agents (PubMed, PMC6711421). Understanding the spatial and functional heterogeneity of the TIME is essential for predicting patient response to immunotherapy and overcoming mechanisms of drug resistance (StatPearls, 2023). Consequently, the TIME is a primary focus of modern oncology research aimed at converting “cold” (immunosuppressive) tumors into “hot” (immunoreactive) ones (Nature Reviews Cancer, 2020).

Mechanism of action

Therapeutic agents modulate the TIME by inhibiting immune checkpoints (e.g., PD-1/PD-L1), blocking pro-angiogenic factors (e.g., VEGF), or neutralizing immunosuppressive cytokines (e.g., TGF-beta) to restore anti-tumor immunity (PubMed, PMC6711421).
