Overview

Tumor immunoglobulin idiotype conjugated to keyhole limpet hemocyanin (Id-KLH) is a personalized therapeutic vaccine designed for the treatment of B-cell malignancies, most notably follicular lymphoma. In these cancers, each malignant B-cell clone expresses a unique surface immunoglobulin with a specific variable region known as the idiotype, which serves as a tumor-specific antigen (Levy et al., 2002, PubMed: 11807399). Because the idiotype is often poorly immunogenic on its own, it is chemically linked to keyhole limpet hemocyanin (KLH), a potent carrier protein derived from the giant keyhole limpet, to trigger a robust immune response (NCI Drug Dictionary). The vaccine is typically co-administered with an adjuvant such as granulocyte-macrophage colony-stimulating factor (GM-CSF) to enhance the recruitment and activation of antigen-presenting cells (Schuster et al., 2011, PubMed: 21969375). This active immunotherapy approach aims to stimulate the patient's own immune system to recognize and eliminate residual malignant B-cells following primary treatment. Despite clinical evidence of efficacy in extending remission, the requirement for patient-specific manufacturing remains a significant therapeutic and logistical challenge.
