The CD40-CD40L (CD154) costimulatory pathway is a critical communication bridge between the innate and adaptive immune systems [StatPearls, 2023]. CD40, a member of the tumor necrosis factor receptor superfamily (TNFRSF5), is constitutively expressed on antigen-presenting cells (APCs) such as dendritic cells, B cells, and macrophages [UniProt, 2024]. Its ligand, CD40L (CD154), is transiently expressed on the surface of activated CD4+ T helper cells [PubMed, 2021]. The interaction between these two molecules licenses dendritic cells to prime CD8+ T cells and triggers B cell proliferation, isotype switching, and germinal center formation [Nature Reviews Immunology, 2017]. In cancer therapy, particularly involving fusion dendritic cells, agonistic targeting of CD40 aims to bypass the need for T-cell help to activate APCs and generate a robust anti-tumor response [Journal of Immunotherapy, 2013]. Conversely, blocking this pathway is a major therapeutic strategy for treating autoimmune diseases like systemic lupus erythematosus and preventing organ transplant rejection [Frontiers in Immunology, 2020]. Clinical development has faced challenges, including thromboembolic risks with early anti-CD40L agents and systemic toxicity with potent CD40 agonists [Lancet Oncology, 2019].
